Alot of the forms are official government documents and lying on them amounts to fraud. Depending on the form and the severity jail sentences aren't unheard of but they are usually only on the security clearance forms. The rest normally just see you being told to stick your job but there are a few exception. Lying about not having any sick days though means feck all in the grand scheme of things.
The criminal record thing really only comes into play if its a sensitive job. If its a job working with kids in sub saharan africa and your on the sex offenders list we will find out even if you try and hide it but if you happened to kill your wife 20 years ago, declare it, and the job isnt a highly stressful job then you might still get that interview. I've seen it happen.
