They don't always reject - people who tell you that are covering up for their inability to pierce them properly. I've had my nape piercing for coming on 15 months and it hasn't moved a millimeter in that time.
Septums are pretty unlikely to migrate unless you're wearing very heavy jewellery. The reason any piercing migrates is that the body decides it is easier to push the jewellery out than it is to heal around it - if there's no pressure pushing the jewellery out then it should heal (infection and poor quality jewellery can cause the skin around the piercing to become irritated and easier to break down and therefore easier for the jewellery to reject).
