I'm guessing that you might have used the 70-300mm for that pic?
If so you probably did get focus (EOS autofocus is good at faces) and it's just a wee bit of camera shake crept in - bigger the telephoto zoom the more shake.
(Although my 70-300 is quite slow auto-focussing sometimes...)
As a rough guide stick 1 over the zoom and try not to hand hold below that speed.
eg. at 70mm use shutter speeds of at least 1/70 second
at 300mm use shutter speeds of at least 1/300 second
If that makes no sense to any newbies using auto or P modes then the rule is that if you are using a long/heavy zoom lens then steady it against a tree/lampost/wall even on fairly bright days!
